Author's text: Patient from Kharkiv, 75 years old: Primary biliary cirrhosis of the liver. Portal hypertension syndrome — grade 2 esophageal varices (photo 1), portal hypertensive gastropathy, splenomegaly. Gastroscopy revealed superficial neoplasia of the lower third of the gastric body, type 0-IIa+c, with a clear demarcation line; prior histology showed poorly cohesive gastric carcinoma G3. Review at CSD confirmed the diagnosis, but more extensive histology is needed for a more precise morphological diagnosis. The patient was referred for CT — apart from the portal hypertension syndrome, nothing else of note. She came in for consultation — EUS was ordered, which showed thickening of the gastric mucosa at the site of the lesion, with a preserved layer between the 3rd and 4th echo layers. Given the concurrent pathology, the patient declined the gastrectomy proposed by the oncologists, and chemotherapy is contraindicated. It was decided to proceed with endoscopic treatment using ESD. Equipment: Olympus EVIS EXERA III, 190 scope HQ, distal attachment, Endoscopy knife Alton Ukraine square knife and IT knife, procedure time 120 minutes, no complications.
Author's text: "For trial purposes, Alton Ukraine introduced the Ball type 2mm knife with fluid delivery. The knife performs well, but the fluid delivery doesn't come out quite centered — it comes out to the side, which requires pressing it deeper into the submucosal layer. We present a case of treatment of a 68-year-old patient with a polypoid tumor in the upper ampullary segment of the rectum. Paris 0-Is, S.Kudo IIIL-IV, JNET 2, size 3.0×2.5 cm. Preoperative histology: tubular adenoma with growth of intramucosal carcinoma. ESD was performed. Olympus EVIS EXERA III, colonoscope 185HL was switched to gastroscope 190HQ during the procedure. Knives: Alton Ball type 2mm and Finemedix Q type. Significant fibrosis on the fold was encountered during the procedure, but it was successfully managed."
